On the one hand, a single patient portal would be an irresistible hacking target.  I do get that.  But wow, am I tired of every doctor and pharmacy I deal with wanting me to create a log-on and individual password.  (I need to deal with three.  Yes I am unreasonably cranky about this.)  Anyone got recommendations for a password manager program?  (Which, yeah, I know, is probably as irresistible a target as that theoretical central patient portal.)

Seconded. It has browser plugins and mobile apps that work well*, and you can set up some emergency coverage by giving another Dashlane user access to specific passwords or secure notes, with or without editing privileges.

*You need a premium subscription to sync across devices; if you only need it on one computer, the free level will work fine.

I also enthusiastically endorse Lastpass. I've used it for years. I can install it on any computer I use regularly, and I use it all the time. I need only remember ONE password (my master password).

I adore LastPass.
EDIT: Get the family version and you can share your passwords with Dragon. I've used LastPass for the last three years at least; it's very stable, and I use it at work, too.

Ditto on LastPass. Also, both Chrome and Firefox will save passwords and logins for you across devices for free. Chrome, of course, is Google and therefore suspect, but Firefox should be safe.

I've had good experiences with both LastPass and 1Password. I work in IT and most IT professionals think the benefits of a password manager far outweigh the risks in most cases (admin passwords for servers are stored in a non-cloud-based safe)--as this article says, there isn't perfect security, but there is better, safer security. https://www.forbes.com/sites/kateoflahertyuk/2019/02/20/password-managers-have-a-security-flaw-heres-how-to-avoid-it/#3c3c1de54e16

And a number of the threats involve physical access to the machine, which for most of us isn't a concern.

And yeah, I feel you on the portals. I honestly try to keep most of my providers in the same health system because then my records (and the fucking portal) are in one place where they mostly know what my deal is already. However, one of the specialties I need isn't available there so I get to have a tour of shitty portals. My favorite was the one that displayed 3 buttons that looked like the login button and the actual login button was in a different position every time-- I'm sure someone thought this was an innovative shell game security measure!

I use KeePass -- it's a portable app, so you can put the whole program on a USB thumbdrive or clouddrive if you want to and be able to access it from any Windows computer.

I use 1Password personally and LastPass at work (I started using 1P with the previous job and just kept with it)
From a security perspective, password managers are a hacking target, but less than you'd think.
Most hackers go after the easy money, and the way the vaults/passwords are stored in the cloud, even if hackers got into the servers, they couldn't easily get access to the vaults.
